Attendance and Continuing Education Information

Please note the fillable pdf attendance certificate will be provided with a QR code on each registrant name badge at the event.  An email will also be sent to all attendees following the event with this information. Attendees can download the form and add their NAME and EMAIL fields for the certificate to become valid. Nothing further is required. The form can be downloaded and saved for attendee records. This information will also be on attendee name badges. 

NYLA CONTINUING EDUCATION

To receive NYLA Continuing Education and /or CTLE credit for this year's conference, attendees are required to:

  1. Attendees are required to submit an evaluation for each program they wish to receive credit for. To complete this, click on the 2026 YSS Evaluation Form Link. You can provide your evaluations in one form or in separate forms.
  2. Attendees are required to submit a CE Request Form to request NYLA/CTLE credit hoursTo complete this, click on the 2026 YSS CE Request Form As NYLA has now transitioned to a new member database, credit will be uploaded into the current NYLA profile of anyone who submits.  Each attendee wishing to receive credit should complete the request form one (1) time.

All fields in the evaluations and the request forms are required. Please do not submit incomplete forms. All incomplete submissions will be deleted.

Attendees will have approx. three weeks (Friday, May 8th) following the conference to submit all program evaluation forms and the CE Request Form. Any requests submitted after the deadline may incur a $10.00 administrative fee.

The office will review all forms following the May 8th deadline and provide further CE information to all who submitted by May 2nd. Questions can be sent to [email protected]
